1915(i) Intake & Community Support Coordinator
Job Overview:
The Ship Group Behavioral Health & Community Services is seeking an organized, dependable, and community-focused professional to join our team as a 1915(i) Intake & Community Support Coordinator.
This is a unique role for someone who is able to balance administrative coordination with direct community-based client support. The person in this position will help manage the provider-side intake process for our 1915(i) Medicaid services program while also working directly with clients in the community to support their goals and progress.
This role is ideal for someone who is detail-oriented, strong with follow-up, comfortable in the field, and able to keep systems, communication, and documentation organized. The position works under the supervision of the agency’s Qualified Professional (QP), who will provide oversight, review, and required sign-off.
What You’ll Do:
This position includes both administrative/program coordination work and direct service work with clients.
Administrative / Intake Coordination Duties:
- Coordinate the provider-side 1915(i) intake process from referral through service start
- Verify and organize referral, insurance, and intake information
- Communicate with care managers, clients, staff, and leadership regarding next steps in the service process
- Track assessment status, required documentation, and service readiness
- Maintain accurate updates in agency systems such as Monday and SimplePractice
- Keep assigned cases organized, current, and audit-ready
- Identify missing items, delays, or compliance concerns and escalate them appropriately
- Help maintain structure and consistency in the day-to-day operation of the 1915(i) program
Community Support / Direct Service Duties:
- Provide approved community-based support services to assigned clients
- Meet with clients in homes, community settings, and other appropriate service locations
- Support clients with goals related to daily stability, independence, housing, community functioning, and service plan progress
- Support clients with employment-related readiness and community-based goal follow-through when appropriate
- Help clients stay organized around practical life needs, appointments, goals, and approved service activities
- Communicate progress, concerns, and barriers to the Qualified Professional
- Complete service documentation accurately and on time

Qualifications:
Minimum Qualifications:
- High school diploma or GED
- Experience in behavioral health, community support, case coordination, social services, housing support, reentry support, Medicaid services, or a related field preferred
- Strong administrative and organizational skills
- Strong written and verbal communication skills
- Basic computer skills and comfort using electronic systems
- Ability to manage both paperwork and field-based responsibilities
Preferred Qualifications:
- Experience working with adults or youth with mental health, substance use, housing instability, functional support needs, or justice involvement
- Experience with scheduling systems, EHR systems, spreadsheets, or task management platforms
- Bachelor’s degree in a human services-related field is a plus, but not required
Transportation / Field Work Requirements: This role includes regular travel in the community.

- A valid driver’s license
- Reliable personal transportation
- Current automobile insurance
- Willingness to travel locally for client visits and community-based work
Schedule:
- Full-time
- Monday through Thursday
- Combination of administrative office time and direct community-based service time
Compensation:
- Starting at $18.00 per hour
- Hourly pay applies to both administrative time and direct-service time
- Opportunity to grow with a developing program
Role Structure:
- This role is part of our 1915(i) provider-side program operations.
- The person in this position will help coordinate intake and service readiness while also providing direct support services to clients.
- This role works under the supervision of the agency’s Qualified Professional (QP), who will provide clinical oversight, review, and required sign-off.
As the program grows, additional leadership and supervisory roles may be added to support expansion, quality, and compliance.
