We need someone who is persistent, organized, and comfortable taking initiative. You should be able to identify what is holding up a referral, follow up with the right people, document progress, and escalate issues when needed.
Responsibilities
• Manage and track 1915(i) referrals from initial contact through intake completion and handoff for services.
• Follow up consistently with clients, families, and care managers regarding appointments, documents, and outstanding steps.
• Coordinate with appropriate assessment, care-management, and clinical teams to support required processes.
• Maintain a clear referral tracker showing each client’s status, barriers, next action, responsible person, and follow-up date.
• Build and maintain relationships with care managers, community partners, and referral sources.
• Help improve intake workflows, outreach practices, and coordination as the program grows.
• Participate in occasional community visits, partner meetings, and outreach activities.
• Report referral progress, recurring delays, and opportunities for improvement to leadership.
• Maintain accurate documentation and protect client confidentiality.
Required Qualifications
• Must meet current North Carolina Qualified Professional (QP) requirements applicable to the population served and assigned services. Education and relevant experience will be verified before hire.
• Direct experience working with North Carolina Medicaid 1915(i) programs or services.
• Experience with intake, referrals, care coordination, or program operations.
• Demonstrated ability to work effectively with care managers and clients.
• Strong organization, phone communication, documentation, and follow-through.
• Ability to manage multiple referrals independently and resolve barriers.
• Ability to work primarily in the office and attend occasional community activities.
